Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Often, treatment can be initiated on the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation, further consultation and discussion with your referring dentist, and second appointment on another day to provide the necessary treatment.

 

 

To ensure a timely appointment, please assist us by providing the following information at the time of your consultation:

 

-PLEASE BRING THE REFERRAL FORM WITH YOU .

 

 

-FILL OUT NEW PATIENT PAPERWORK, PRINT AND BRING WITH YOU TO YOUR APPOINTMENT.

-Bring a list of your current medications.

 

-If you have dental insurance, please call our office in advance and provide us with the insurance information. At the time of your appointment, bring the necessary information. This will save time and allow us to help you process any claims.

 

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

 

 

Cancellation Policy:

 

If you are unable to keep your appointment and need to reschedule, please call (do not email) our office and give us a 24 hours notice. Patients who fail to do so, will be charged a cancellation fee of $50.
